Mandatory Forms
Below are the required DocuSign forms that you must fill out, sign, and submit prior to your arrival on campus. Pre-College students must complete the forms with a parent or guardian.
In order to access these mandatory forms on DocuSign, you must first activate your USC Account. For instructions on how to activate your USC Account, go to USCard & Accounts.

- Parental Consent for Medical Treatment - This is different from the USC Student Health Medical History and Consent for Medical Treatment form.

Medical Forms
- Once you have created your USC Account, please log in to the USC Student Health Portal at MySHR and submit the USC Medical History and Consent for Medical Treatment form.
- The Medical History and Consent for Medical Treatment form must be uploaded directly to the USC Health portal at MySHR. This document must not be emailed to the SCA Summer Program.
International Forms
- International students must have digital copies of all relevant immigration documents that are listed on the Accepted International Students Page.
